All Paramus Recreation Commission staff members must fill out the appropriate payroll paperwork. These forms include the I9 and Federal W4. Staff members under the age of 18 and still in high school also need to fill out working papers. Staff members over the age of 18 will be subject to an annual background check. All staff members will also be required to complete an annual drug test.

The I9 form (PDF) is used to verify a person's employment eligibility. Pages 1 through 6 of this form are informational and important to read through. Section 1 on Page 7 must be filled out by the employee after they are offered a job. Section 2 on Page 8 will be filled out by the employer. Page 7 should both be turned in to the employer.
Documentation is also required with the I9 form. Please see acceptable forms of documentation on Page 9. You may provide one document from List A (i.e. passport) or one document from both List B and List C (i.e. driver's license and social security card). If this documentation is not included with Page 7, the form will not be accepted. Please only return page 7 and your identification.
Federal & NJ W4 Form
The Federal W4 form (PDF) is a payroll form that is used to determine a person's tax withholdings. The top portion of this form includes a worksheet to help determine your allowances; this number would be entered in Box Number 5. If you are exempt from tax, you would write "exempt' in Box Number 7. Please be sure to sign this form and include your social security number. If you need assistance filling out this form, please speak with a professional, as we are not qualified to tell you what your allowances/withholdings should be.
New Jersey W4
The NJ W4 form (PDF) is very similar to the Federal form, please fill out with your withholding information. If you are exempt from tax, you would write "exempt" in Box Number 6.
Drug Test Form
All staff members must complete an annual drug test. Drug tests must be completed at the following location:

Those going for a drug test must fill out and bring the Drug Test form (PDF). There is a $45 fee due to Doctors Express Urgent Care for the test. Results will be sent to Borough Hall, you do not need to bring anything to Recreation. Please keep the copy of your receipt. If you earn less than $500 throughout the year, this drug test fee will be reimbursed, but you will need to provide the receipt.
All staff members under the age of 18 and still in high school are required to complete working papers. Working papers can be obtained at your school's main office. The parent will complete Section A. The employer will complete Section B. The school nurse or employee's doctor will complete Section C. The school's main office will complete the rest of the form. After the form is finished, the employee must bring the copy back to the employer. An online copy of working papers are not available, however, parents can read more information online (PDF).
